Assessment
Before making a decision about the needs of an individual, it is important that a thorough assessment of the current circumstances and environment be completed. This is done by an experienced Care Manager using assessment tools. Getting to know the individual’s environment, functional abilities, medical condition and family/social support system establishes the foundation for a needs outline which allows for planning and implementing necessary services.
Care Management
This program is always structured around the client’s needs, preferences, personal values, and financial capabilities.
Care Management includes ongoing evaluation of environmental safety issues, medication use & effectiveness, transportation, advocacy with service providers, caregiver supervision, and making sure social & spiritual needs are being met..
Any and all of the above activities can be included in our complete monitoring service. This is especially helpful for families that live out of area. Monitoring provides ongoing assessment for changes in the client’s status.
Supportive Consultation
Too often a family member’s illness or aging requires us to deal with issues most of us have not planned for. Unfortunately, the issue often presents itself very suddenly leaving the family feeling confused, frustrated and overwhelmed.
